I-9 with Supporting Documents

Federal law requires 1-9 be completed within three days of employee’s start date.

W-4 with Copy of Social Security Card (or ITIN#) OR W-9 with Copy of Social Security Card

Signed Application for TEAMS employees

The applicant or department should print a copy of the application; it should be signed by the employee and submitted as part of the payroll paperwork.

Signed Copy of OPS Application

If the applicant applied online, he/she or the department should print a copy of the application; it should be signed by the employee and submitted as part of the payroll paperwork. If no online application was completed, a signed copy of the paper OPS application.

Education and Experience Verification Form

Notice of Appointment (Contract)

While dates of an employee’s first UF Notice of Appointment may be six or twelve months, it is recommended that the first Notice of Appointment issued for a newly hired exempt TEAMS employee be for six months. The Notice of Appointment is NOT required for newly hired nonexempt TEAMS employees; they serve 6-month probationary periods, after which they should be issued Notices of Appointment for one year.

Criminal Background Check

Please work with your recruiter in Recruitment and Staffing, Office of Human Resource Services, (352) 392-2477, in the event a criminal background check is required as part of your hire process.

Driver’s License (Copy)

A copy should be submitted in the event it is part of the required I-9 documentation or in the event the employee will be expected to drive a state vehicle.

Emergency Contact

Employee should complete either http://www.hr.ufl.edu/recruitment/forms/emergency.pdf or go online at my.ufl.edu, then following the navigation >My Account > Update Emergency Contact to provide this information.

Health Assessment (Completed Online)

If a health assessment is required, it must be completed and approved prior to the employee's first day of work.

Licenses, Other Applicable

Please submit copies of licenses if required by the position. Examples include licenses required for Physician Assistants, ARNPs, Nurses, Dental Assistants and/or Dental Technicians.

Nepotism—Approved Waiver

 

Selective Service Registration (Male, 18-25)

Departments may verify selective service registration at https://www.sss.gov/RegVer/wfVerification.aspx. Please print a copy of the verification and include with paperwork.

Special Status Letter (Time-Limited or Temporary)

Work Permit

Submit Foreign National Information Form to Tax Services, PO Box 113203, to Generate Windstar Packet. Submit Forms Generated to Appropriate Core Office I-20 (F1 Visa), DS-2019 (J1 Visa), or 1-797 & Labor Condition Application (H-1B) (as provided)
