Important information regarding your W-2
Check the myUFL directory to ensure your information is accurate
Many employees are not receiving important personnel information because their addresses are incorrect in UF’s directory. The UF directory is the sole authoritative source of employee addresses and is used to generate mailings for W-2s, benefits information, and other important campus-wide updates. Because departments are not required to correct employees’ contact information in the UF directory, all employees are responsible for correcting and updating their own contact information.

Domestic partner insurance open enrollment begins January 9
Open Enrollment for domestic partner insurance will run from January 9 through 31. Benefits-eligible employees are now eligible to enroll in the new health option, which provides family coverage for both same- and opposite-sex partners, as well as dependent children, as long as partner/dependent criteria are satisfied. The current plan is administered through AvMed and based on an HMO plan design, while offering expanded choices for provider coverage. Employees will be able to seek coverage both within and outside the local AvMed network; however, benefits tend to be less generous for services as one moves outside the local network. For additional details regarding plan provisions, tax implications for partner coverage, upcoming information sessions, and an online tutorial please visit our web site at www.hr.ufl.edu/benefits/domesticpartner.htm.
To enroll, please visit University Benefits and Retirement in Room 422 of the Stadium by January 31, 2006. You (and your partner) will be asked to complete an enrollment form and sign an affidavit that indicates you both satisfy the eligibility requirements for the benefit. If you have questions, please call University Benefits and Retirement at 392-1225 (SC 622-1225). |
