PostDocCare

PostDocCare is an injury and sickness insurance plan underwritten by Florida Blue and created specifically for Postdoctoral Associates and Postdoctoral Fellows as well as their dependents.

**Notice regarding additional insurance id cards
Florida Blue does not issue separate ID cards for dependents under your insurance plan. You will receive two ID cards under your name and dependents will use your insurance card information. You may order additional cards by calling the Florida Blue customer service number on the card or by creating an online account at www.floridablue.com and clicking the additional cards link.

**Notice regarding UFSelect Voluntary Benefits
Postdoctoral Associates and Postdoctoral Fellows are eligible for UFSelect plans.  UFSelect plans are voluntary post-tax benefits that are sponsored by the University of Florida and will include dental, vision, accident, critical illness, supplemental hospital, life events, pet and legal insurance.  The premiums for all UFSelect plans are employee paid through payroll deduction or direct bill depending on the employee’s classification.  To learn more about UFSelect, please visit http://www.fbmclearningcenter.com/uf.  Current Postdoctoral Associates and Postdoctoral Fellows can enroll in UFSelect Voluntary Benefits during Open Enrollment (October 8th through November 2nd) for a January 1, 2013 effective date.  During Open Enrollment, employees are encouraged to schedule a meeting with an FBMC enrollment counselor online or by calling (866) 998-2915. You may also sign in to myUFL with your GatorLink login and password, then navigate to Main Menu > My Self Service > Benefits > UFSelect & GatorCare Benefits to enroll online through FBMC's Premier Enroll system, which will take you step by step through the process.  For new employees hired during the Open Enrollment period, you may sign up for UFSelect plans, however coverage will not go into effect until January 1, 2013.

All University of Florida employees on an appointment as a Postdoctoral Associate or Postdoctoral Fellow may enroll online at www.hr.ufl.edu/benefits/post_doc_care/enroll.asp within 30 days of the employee’s date of hire or during the specified open enrollment period.  Eligible participants who do enroll may also insure their spouse or registered domestic partner and/or children.  Dependent coverage must be elected at the time the employee enrolls.   

Supporting documentation including a marriage certificate for spouse coverage and/or birth certificate for child coverage must be provided within 31 days of the coverage effective date for new hires or within 31 days of the close of the QSC event window for Qualifying Status Changes.  Postdoctoral Associates or Postdoctoral Fellows who wish to enroll a domestic partner will need to contact the GA and Post Doc Associate Benefits Office at 352-392-0003 in order to complete the appropriate paperwork.

Eligibility
University of Florida employees on an appointment as a Postdoctoral Associate or Postdoctoral Fellow may participate in the PostDocCare Health Insurance Plan. To be eligible, a Postdoctoral Associate or Postdoctoral Fellow must be on a Postdoctoral Associate or Postdoctoral Fellowship appointment through the University of Florida. The employing department must provide documentation of the Postdoctoral Associate appointment or Postdoctoral Fellowship.  Eligible Postdoctoral Associates and Postdoctoral Fellows must enroll within 30 days from the date of hire.  Enrollment must be completed within 30 days of the employee’s date of hire regardless of the Post Doc’s status in the PeopleSoft system.  The PostDocCare enrollment can be submitted without an active job showing in PeopleSoft.  Coverage will be effective the first of the month following the date of enrollment.  Dependent coverage must be elected at the time the eligible Postdoctoral Associate or Postdoctoral Fellow enrolls.  Eligible Postdoctoral Associates or Postdoctoral Fellows who do not enroll within 30 days from the date of hire can enroll and/or add/remove dependent coverage during the open enrollment period that will take place concurrently with the state of Florida benefit open enrollment period in the fall.

Eligible Postdoctoral Associates and Postdoctoral Fellows who do enroll may also insure their dependents.  Eligible dependents are the spouse or domestic partner and their children under 26 years of age.

Costs
There is no cost to the Postdoctoral Associate for PostDocCare coverage; coverage for both the Postdoctoral Associate and covered dependents is paid for by the University of Florida.  Departments will need to use the appropriate Pooled Fringe Rate to budget for benefit costs.  

The PostDocCare premium and the premium for any elected dependent coverage will be paid according to the funding grant and its specifications for employees meeting the PostDocCare eligibility requirements as a Postdoctoral Fellow.   The actual health insurance premium cost will be charged directly to the funding grant.  The monthly rates for the 2012-2013 policy year are as follows:

Please visit www.floridablue.com and login to MyBlueService.  Access will be available once your eligibility has been confirmed by the University of Florida and your coverage updated with Florida Blue.
