Disability (includes sickness): $400-$5,000/month according to income, level of coverage selected, and elimination period selected.

Comments

May not exceed 2/3 of salary.
Disability benefits may be payable from first day of injury and for base plan are payable for no more than 24 months.
If you are 64 years of age or older or live outside the United States you do not qualify for this policy.
