Colonial Life's Accident Insurance/State of Florida
This coverage offered through the State of Florida assists employees to pay for the following expenses when injured during covered accident:
- 24-hour coverage on or off the job
- Medical treatment for broken bones and dislocations, or physical therapy
- Crutches, wheelchairs, or other medical aids you may need as a result of your accident
- Copays and deductibles
To enroll:
- Enroll online via the People First (PF) web site
- Call PF at 1-866-663-4735
- Mail your form or fax to PF at (904)828-6092
In addition to your People First enrollment, you must also submit a company application and call a Colonial Representative at (352) 335-5016 to complete the enrollment process. Be sure to retain a record of your transactions.

UFSelect/Trustmark Accident Insurance
The University of Florida offers Trustmark Accident Insurance through UFSelect Voluntary Benefits Program. These insurance help employees to pay when injured in an accident and offers:
- 24-hour coverage on or off the job
- Includes coverage for employee, spouse/domestic partner, and children
- Benefits paid to employees regarding of any other coverage
- Initial care benefits
- Injury benefits
- Accidental death
- Catastrophic accident benefits

UFSelect/Standard Long-Term Disability Insurance
Voluntary disability insurance from Standard Insurance Company is designed to pay an income benefit in the event of a covered illness or injury. The coverage includes:
- 66 and 2/3 of the first $22,500 of an employee pre-disability earnings, reduced by deductible income
- Minimum monthly benefit is the greater of $100, or 10% of any LTD benefit before reduction by deductible income
- Benefits pay from the end of the elimination period until Social Security Normal Retirement Age (SSNRA) as long as an employee meets the definition of disability as specified in the policy
- Cost living adjustment benefit of 2% for five years
- Assisting living benefits
- Survivor's death benefit
