Coverages

Choose any amount of insurance in increments of $10,000 up to $750,000.
Principal sum may not exceed 10 times annual salary of employee for selections in excess of $350,000.

Comments

Employee may enroll or make changes to coverage at any time.
Employee must have .50 FTE or higher to enroll.

Dependent coverage is available from birth to age 19 (thru age 25 if attending an institution of higher learning on a full or part time basis) and primarily dependent on the insured for support and maintenance. Disabled children incapable of self-sustaining employment and who are primarily dependent on the insured for support and maintenance  may continue beyond the age limit as long as the policy is in force,  However, they must be  both enrolled and had sustained their disability prior to the age of 19 to stay on the plan.

Refer to plan brochure for more details or contact the Benefits office.  Additionally, former dependents who are no longer  eligible such as an ex-spouse OR if family coverage is no longer needed, it is the employee’s responsibility to notify the University Benefits Office to complete the appropriate forms to cancel coverage.

Please note; Claims will not be processed/paid for dependents who do not meet the eligibility criteria at the time the application is filed.

How to Enroll

  • Complete an enrollment form and return it to the Benefits office by campus mail to P.O. Box 115007; fax to 352-392-5166; or take it to the Human Resource Services satellite office nearest you.
  • Attend the UF Benefits’ Group Enrollment Session for assistance with completing your enrollment in addition to other plans.

Be sure to retain a record of your transactions.

Accident:
24-hour accident coverage on or off the job
Plan pays benefits for specific care on accidental injuries or dismemberment according to schedule. Please see brochure.

Comment

Disability benefits may not exceed 2/3 of salary.
Based on coverage selected, benefits may be payable from first day of disability through no more than 24 months.

How to Enroll

Choose one of the following methods to enroll:

  • Enroll online via the People First (PF) web site.
  • Call PF at 1-866-663-4735 to enroll or make a change by phone.
  • Mail your form or fax it to PF at 904-828-6092.
  • Attend the UF Benefits’ Group Enrollment Session for assistance with completing your online enrollment with People First.

In addition to your People First enrollment, you must also submit a company application and call a Colonial Representative at 352-335-5016 to complete the enrollment process. Be sure to retain a record of your transactions.

Long-Term Disability Insurance: STANDARD

Coverages

66 2/3% of salary up to $270,000. Benefits are paid beginning on 91st or 31st day of disability depending on the plan elected.

Effective 9/1/2011:  The monthly maximum benefit will increase up to $15,000.  If approved for LTD, your monthly benefit is based on 2/3 salary up to the new maximum benefit limit.   Employees earning > $270,000 would receive the max. monthly benefit of $15,000.

Comments

Must be .50 FTE or more to enroll.
Benefits reduced for primary social security, other disability insurance, salary or paid leave, workers' compensation, or retirement.
Minimum benefit paid is 10 percent of regular benefit or $100, whichever is greater. Enrollment available first 60 days of employment and by medical application thereafter.
