State of Florida Basic Group Life Insurance

  • 100% Employer paid for full-time staff; Cost for Part –time based on FTE.
  • Refer to People First on-line enrollment screen or the People First Service Center for actual cost.

  • $25,000 coverage amount for all employees.  For age 70 and older, the 50% reduction will no longer apply and the maximum coverage will be $25,000 which is the same for all employees.      

    Available for employee only.

State of Florida Optional Group Life Insurance

Current Plan: Available in multiples up to seven times annual salary OR $1 million, whichever is less.

  • Medical Application is required

How to Enroll

Choose one of the following ways to enroll:

  • Enroll online via the People First (PF) web site.
  • Call PF at 1-866-663-4735 to enroll or make a change by phone.
  • Mail your form or fax it to PF at 904-828-6092.
  • Attend the UF Benefits’ Group Enrollment Session for assistance with completing your online enrollment with People First.

Be sure to retain a record of your transactions. If an early effective date is requested for new hires only, for the basic life plan, please contact the benefits office at 392-2477 regarding payment of premiums.

First 60 days of employment

Fixed term. Offers coverage for the employee, spouse, and child. Guarantee issue amount up to $300,000. Optional spouse and child coverage for 1/2 issue of employee; maximum allowable for child coverage is $25,000.

Annual open enrollment

During open enrollment 2011, options to consider:

  • This year only, employees can enroll for up to $10,000 in coverage as a guarantee issue (without medical application)
  • $5,000 for spouse and dependents, subject to 50 percent of employee coverage, and maximum plan limits up to $25,000 on the child.

Comments

Can apply via medical application at any time or for amounts over guaranteed renewal amounts during open enrollments.

Eligible dependents must meet the definition under the U.S. IRS Code and not be serving in the armed forces.* This policy covers unmarried children from 14 days old* to age 19.  Coverage may continue from age 19 through 25 if enrolled as a full time or part time student in an accredited educational institution.  Dependents totally and permanently disabled and who became disabled prior to reaching age 19 may also continue coverage on the plan as long as the policy has been continuously in force and they were enrolled before age 19.

Additionally, former dependents who are no longer  eligible such as an ex-spouse OR when the last child no longer qualifies for your dependent plan, it is the employee’s responsibility to notify the University Benefits Office to complete the appropriate forms to cancel coverage.
Please note; Claims will not be processed/paid for dependents who do not meet the eligibility criteria at the time the application is filed.

  • Attend the UF Benefits’ Group Enrollment Session for assistance with completing your UF Term Life enrollment in addition to other plans.
