Enrollments for students and dependents must be completed by the enrollment period deadline (see Enrollment Section for deadlines).

Changes to coverage must also be made prior to the enrollment period deadline. This includes adding/removing dependents and/or changing coverage terms. Refunds are only granted if eligibility is not met or upon entry into the armed forces. 

A dependent (spouse or child) can be added to the policy at any time if there is a change in status (marriage, adoption, birth, etc.) and full premium is received (pro-rating will be not allowed). The effective date for dependent coverage added after the enrollment period deadline is the date that the full premium is received by the insurance company. If dependent coverage is added due to the birth of a child, the effective date is the child's date of birth as long as the request to add coverage is made within 30 days of the child's birth. You must contact the GA Benefits Office at 392-0003 or gabenefits@admin.ufl.edu to add newborn coverage within 30 days of the child's birth.

Important Information for Graduate Students not registered for summer classes and enrolled in Annual or Spring/Summer (2008) coverage under GatorGradCare

Graduate students who are not registered for courses during the summer and are enrolled in annual or spring/summer coverage together under GatorGradCare may still use the Student Health Care Center during the summer months. Graduate students falling into this category have the following two options in order to utilize the Student Health Care Center during the summer months:

  • The GatorGradCare graduate student with annual or spring/summer coverage may pay the summer health fee, which covers office visits and use of the Student Health Care Center Pharmacy. The health fee cost for the summer are as follows:
  • Summer A: $75
  • Summer B: $75
  • Summer C: $150
    (Entire Summer)

The health fee is paid directly to the Student Health Care Center.

The graduate student’s office visit would be covered by this Health Fee. Once the health fee is paid, the Student Health Care Center benefits under GatorGradCare would be in place. A description of these benefits can be found on page 8 of the GatorGradCare brochure or by calling Student Resources at 1-800-996-4698.

If a GatorGradCare graduate student has purchased coverage for a spouse or domestic partner, then both the graduate student and the spouse would have to pay the health fee in order for the spouse or domestic partner to be seen at the Student Health Care Center.

  • The GatorGradCare graduate student with annual or spring/summer coverage may be seen at the Student Health Care Center without paying the summer health fee. The Student Health Care Center benefits under GatorGradCare would be in place. A description of these benefits can be found on page 8 of the GatorGradCare brochure or by calling Student Resources at 1-800-996-4698. An additional $60 fee will be added to the graduate student’s total bill. This will make the student’s 20% coinsurance amount slightly higher.

Example: If a student’s total bill was regularly $100, the student would pay 20%, which equals $20. However, an additional $60 will be added to the total bill per visit. So, the total is now $160. The student will pay 20% of $160, which equals $32.

The GatorGradCare graduate student with annual or spring/summer coverage may also use the Student Health Care Center Pharmacy without paying the summer health fee. An additional $10 fee will be added to the cost of the prescription. The student would still pay the appropriate copay under GatorGradCare for the prescription. Students will only notice this extra fee if the cost of the prescription was usually less than the $10 minimum copay.

This is not an option for spouses or domestic partners covered under GatorGradCare.

These options do not apply to students who are registered for summer classes and are covered under GatorGradCare.

Enroll in GatorGradCare for the following coverage terms: 

Summer Only: May 16, 2008 – August 15, 2008

Annual: August 16, 2008 – August 15, 2009

Fall: August 16, 2008 - December 31, 2008

Fall/Spring: August 16, 2008 - May 15, 2009

The enrollment period for summer only will begin March 2008 and will continue through May 16, 2008. This plan is for students who did not previously enroll in the Annual plan or the Spring/Summer plan.

The enrollment period for Annual, Fall/Spring, and Fall only coverage will begin March 2008 and will continue through September 14, 2008.  All eligible graduate students who wish to enroll in GatorGradCare for the 2008-2009 Academic year, must submit an enrollment for one of theses plans.

Eligibility

University of Florida graduate students on an appointment as a graduate assistant, teaching assistant, or research assistant may participate in the GatorGradCare Health Plan beginning January 2007. To be eligible, University of Florida graduate students must be enrolled in a graduate degree program, on an appointment through University of Florida, appropriately registered and appointed 0.25 FTE (full-time equivalent) or greater for a particular semester.

University of Florida graduate students on a Pre-doc Fellowship appointment and appropriately registered, are also eligible to participate. There is no FTE requirement for graduate students on a Pre-doc Fellowship appointment. Criteria for appropriate registration can be found in the Registration Requirements section of the Graduate Catalog.

In order to ensure that graduate assistants or pre-doc fellows meet the above eligibility criteria, departments must ensure the following:

a. The TA, RA, or Pre-doc Fellow appointment must occur via the Letter of Appointment (LOA) process in PeopleSoft.
b. The student receives a stipend as a bi-weekly paycheck from the appropriate UF account.
c. The student receives a tuition waiver from the appropriate UF account.
d. Health insurance premium is paid from the appropriate UF account.

Insurance ID cards will be mailed by Student Resources after eligibility is confirmed. Eligibility will be checked after drop/add is completed.

Once enrolled in GatorGradCare

Graduate Assistants and Pre-doc Fellows participating in GatorGradCare need to visit the Student Health Care Center first, and obtain a referral for treatment that cannot be provided at the SHCC. Spouses and children that are covered under the plan do not need to obtain a referral from the Student Health Care Center. The referral requirement is waived for the following reasons:

a.  Medical Emergency.
The student must return to Student Health Care Center for necessary follow-up care.
b.  When the Student Health Care Center is closed.
The student must return to Student Health Care Center for necessary follow-up care.
c.  When service is rendered at another facility during break or vacation periods.
The student must return to Student Health Care Center for necessary follow-up care.
d.  Medical care received when the student is more than 35 miles from campus.
The student must return to Student Health Care Center for necessary follow-up care.
e.  Medical care obtained when a student is no longer able to use the SHCC due to a change in student status.
f.  Maternity
g.  Psychotherapy

International Graduate Assistants and Pre-doc Fellows needing to provide insurance verification to the International Center for registration purposes, may submit the e-mail confirmation addressed from webmaster@uhcsr.com to the International Center at insurance@ufic.ufl.edu.

Total cost of individual health insurance provided for eligible graduate students

Note: If the eligible graduate student is paid from grants and/or auxiliaries, funding for the individual plan cost will come from those resources.

  • Graduate students (TA or RA) may choose to cover a spouse, partner, or child. The dependent premium must be taken through payroll deduction.
  • Graduate students on a Pre-doc Fellowship may choose to cover a spouse, partner, or child. There is no payroll deduction option for graduate students on a Pre-doc Fellowship appointment as has been the current practice. The dependent premium must be paid directly to the vendor by the student.

Agents are available Monday through Friday 6:00 a.m. - 7:00 p.m. Once eligibility is confirmed, students will be able to set up an online account with Student Resources. This account will allow students to request and print ID cards, view claim status, and other insurance information. 

Students can create an account at www.uhcsr.com by selecting “Create Your Online Account Now ” on the right side of the home page. Follow the onscreen prompts - enter your first name, last name, date of birth, and enter your member id number or your email address to access account information. Please note that ID cards and coverage information will not be available until after eligibility has been confirmed (at some point after Drop/Add).

GatorGradCare Maternity Coverage Information

Benefits will not be paid for students or spouses of students who are already pregnant prior to the effective date of coverage through GatorGradCare. However, this exclusion will be waived for students and/or spouses of students who were covered previously by a group insurance plan (through another employer, etc.). In order for the exclusion to be waived, students and/or spouses of students must provide proof of continuous coverage with a comparable plan for at least 12 consecutive months prior to the effective date of coverage. Proof of continuous coverage can be obtained by contacting your previous insurance provider and asking for a Certificate of Credible Coverage. This document should include the names of the insureds covered under the policy as well as coverage effective and termination dates.

If a student or a student's spouse is pregnant and the conception occurred while covered under a student insurance policy (through UF or another school, etc.) prior to GatorGradCare, then that prior student insurance policy would include the extension of benefits for the pregnancy that is required by Florida Insurance Code 627.667.

If a student or a student 's spouse is already pregnant prior to the effective date of GatorGradCare coverage, and there was no previous coverage for a consecutive 12 months prior to the effective date of GatorGradCare coverage, then the maternity and delivery will not be covered by GatorGradCare.

If a student or a student’s spouse becomes pregnant while covered under GatorGradCare, then the maternity and delivery will be covered by GatorGradCare.

If you need to submit a Certificate of Credible Coverage to the insurance company, you can fax the documentation to 469-229-5662 once you have been confirmed eligible for GatorGradCare for the semester that you have submitted an enrollment.
