Checklist for Salaried Faculty Terminations/Resignations
Termination/Resignation. Submit an electronic ePAF Form. Attach the letter from the employee stating his/her last date of employment or the letter of appointment indicating “No further notice of cessation,” or the letter of non-renewal that states the last date of employment. If processing a future resignation, take into consideration accrual and usage of leave prior to the last date of employment. NOTE: If processing an appointment after the fact and the employee has been overpaid, the department will have to do an adjustment to the Central Leave System to correct the over accrual on the leave and will need to work with Payroll regarding a cash refund. Leave cash outs are processed by Departments.
Retirement. Submit an electronic ePAF Form. Attach the letter from the employee stating his/her last date of employment. If the Department has voted on the granting of the Emeritus title and approved by the appropriate administrator (senior vice president or provost), please input reason as “retire with Emeritus status.” Otherwise, please use “normal retirement” until Emeritus status is approved. Forward approved letter to HR/APO Core Office and changes will be made accordingly. Leave cash outs are processed by Departments.
Deceased. Submit an electronic ePAF only. No other paperwork required. Leave cash outs are processed by Departments.
