Checklist for OPS Faculty Status Changes

Increase or decrease in FTE: Submit an electronic ePAF Form along with letter of justification for change approved by appropriate administrator.

Change in Department ID : Submit an electronic ePAF Form only. No other paperwork required.

Title Change: Submit an electronic ePAF Form along with letter of justification approved by appropriate administrator. Prepare letter to faculty member notifying him/her of title change and effective date.

Salary Increase: Submit an electronic ePAF Form along with letter of justification approved by appropriate administrator. Prepare letter to faculty member notifying him/her of salary increase.

Name Change: A copy of W-4 card (Employee’s Withholding Allowance Certificate) and Social Security card showing new name (must have UFID# written on both documents.)

Employment Authorization (Visa) Extension For Foreign Nationals: When processing an employment authorization (Visa) Extension submit the correct visa documents with the new Visa information and a new or updated I-9.
